What Is the GUIDE Model?

A Medicare Innovation for Dementia Care

The GUIDE Model is a voluntary program launched by the Centers for Medicare & Medicaid Services (CMS) in 2024. It is designed to help people with dementia remain safely at home longer by improving the coordination of their care and providing direct support to the family caregivers who assist them.

Participating healthcare organizations — called GUIDE participants — receive Medicare payment to deliver a comprehensive package of dementia care services, including a dedicated care navigator, 24/7 support access, and caregiver respite.

Caring for someone with dementia is among the most demanding caregiving experiences. The GUIDE Model specifically recognizes the burden on family caregivers and includes respite care as a core benefit — helping caregivers take necessary breaks without sacrificing the safety or wellbeing of their loved one.


What Services Are Included?

Care Navigator
A dedicated care navigator helps coordinate all aspects of the person's dementia care — connecting them to medical, social, and community services so caregivers aren't left to figure it all out alone.
Caregiver Respite
The program includes coverage for respite care services — giving family caregivers planned breaks of up to a defined number of hours per year, covered under Medicare.
24/7 Support Line
Caregivers can access around-the-clock support by phone for guidance during nighttime crises, behavioral changes, or urgent care questions — without a trip to the emergency room.
Caregiver Training
Structured education and training for family caregivers on how to safely manage dementia-related behaviors, communication challenges, and daily care tasks.
Care Planning
A comprehensive, individualized care plan is developed for the person with dementia and updated regularly, ensuring that care goals align with the person's values and preferences.
Community Connections
Referrals to community-based services such as adult day programs, Meals on Wheels, transportation assistance, and local dementia support groups in the Tulsa area.

Who Is Eligible?

To receive GUIDE services, the person with dementia must be enrolled in traditional Medicare (Parts A and B) and have a diagnosis of dementia. They must not be enrolled in a Medicare Advantage plan, hospice, or PACE program at the time of enrollment.

How to Access GUIDE Services in Oklahoma

GUIDE services are delivered by healthcare organizations that have enrolled in the CMS GUIDE Model. Your loved one's primary care physician or neurologist may participate, or can refer you to a participating organization.
